Important information for scheduling your Alpine PT appointment online:

Welcome to Alpine Physical Therapy's Online Scheduling! Please read through the information below prior to booking your appointment.

Alpine PT cannot accept online scheduling for new patients who are covered by Medicaid, Workers Compensation or Motor Vehicle Accident claims. If you are a new patient covered by Medicaid, Workers Comp or an MVA claim,  please call one of our clinics to schedule. 

If you would like an appointment urgently and you don't see an appointment time that meets your needs online, please call one of our clinics listed below to see if we have any short notice availability.

If you are scheduling a visit for one of our specialty services including Concussion, Vestibular Lymphedema, Pelvic Health, or Occupational Therapy please make note of the appropriate practitioner and location for that specialty from the list below. You will be prompted to enter your requested practitioner and location once you are in the Scheduling platform. You can also call one of our clinics to schedule specialty care to make sure you are matched with the appropriate provider.

Vestibular: Alpine South - Brace Hayden

Instructions for scheduling:

1.  Select ‘Schedule Now’ button below
2.  Enter requested information from Prompt EMR to enter the scheduling portal
3.  For NEW PATIENTS, click ‘Create New Case’ at the top right corner of the page and follow instructions.  The following documents and information will be requested to schedule a new evaluation online:
  • Drivers license 
  • Insurance card (if choosing to bill to insurance)
  • Referring Physicians Name (if applicable)
  • Prescription/Script for Physical Therapy (if applicable)
  • Any other related files such as medication list or op reports can also be added
4.  For EXISTING PATIENTS, be sure the correct case for your care is listed and then choose your preferred appointment day and time.
5.  Once you have requested your appointment, our team will review the request and send confirmation for the appointment as soon as possible. If you request an appointment in the evening or over the weekend, we will reply with confirmation on the following business day.
